As a Primary Care Health Network, we are committed to the principle that all the residents of the Isle of Dogs should have access to a good quality of life, through improved health & well-being, equitable & coordinated access to local primary & community health services, social care  & community support services.

 

Our aims and objectives

Continue to establish robust and sustainable partnerships across sectors & organisations and with local residents to:

Ensure we deliver patient/user-focused services delivering Equitable Outcomes, Choice, Quality, & Improved access & increased satisfaction

Re-design health & social care services and user pathways to ensure seamless, coordinated and where possible,  locally delivered services.

Improve patient/user engagement to co-create and deliver appropriate, responsive and effective services

Promote healthy lifestyles for Isle of Dogs residents

Ensure patients with long term conditions are pro-actively managed and empowered to self-care.

 
 

Our Area of Tower Hamlets

 

The area has undergone many changes in the last two hundred and fifteen years. These include the following historical factors:

 
 

The building of secure docks to trade with the rest of the world

 

The development of heavy industry and shipbuilding

 

Decline and then closure of the docks in the 1970s and the loss of industry

 
 

The arrival of the London Docklands Development Corporation and then the economic centre at Canary Wharf

 
 
 
 
 

The arrival of the tallest and densest residential buildings in the UK

 

Our Network has been in operation for over 10 years and over this time has been at the forefront of many important changes to the health and wellbeing of local people.

Excellent Clinical achievement matched by work in the community around self-health has been a hallmark of Network success.

With the formation of Primary Care Networks (PCNs) across the country, we see this as an opportunity to consolidate and enhance our work and to refocus our plans.

Hence our  New Wave of Health

Moving forward we will concentrate on:

  • How we prepare for the growth of the population and the new community this brings?

  • With some of the highest rates of Low Mood and Anxiety in the Borough – how we create, maintain and deliver quality services to these community members?

  • How do we attract new Clinicians (GPs and others) to work on the Isle of Dogs?

  • What can we do to prevent poor health (pro-active not just reactive) when our GP waiting rooms and hospital waiting lists are full?
